New patients are requested to complete this form to provide Adelaide Allergy with all relevant information.
Initial Consultation
Your initial consultation is dedicated entirely to understanding your health history, symptoms, and concerns. Our GP Allergists will take the time to talk through your medical background, previous reactions, family history, and lifestyle factors that may be contributing to your symptoms. This thorough discussion allows us to identify exactly which allergens or conditions to investigate.
No testing will done at the initial consultation.
Pricing
Initial consultations, skin prick testing, review appointments and telehealth appointments all attract an out-of-pocket fee of $240.00
If your initial consultation needs to be conducted via telehealth you will not be eligible for a Medicare rebate, as per Medicare

Referrals
We welcome everyone at Adelaide Allergy and referrals are not required for most of our services including Allergist appointments, some testing or internally referred services. A referral may help you get higher rebates from Medicare and we strongly encourage you to bring a referral from your GP if possible. We believe that collaborative care leads to the best outcomes, and a referral allows us to keep your GP informed and involved in your ongoing care. Most importantly, we want to make accessing allergy care as easy as possible because your health and wellbeing matter to us.
